
The Trade Union Conference on the Future of Community Health Workers (CHWs) in South Africa brought together unions, CHWs, health experts, and civil society to address the precarious working conditions of CHWs and strategize for their full recognition within the public health system.

While the government’s pledge to permanently employ 27,000 CHWs marks a major union victory, thousands remain at risk due to donor funding cuts, austerity measures, and unclear roles under the new National Health Insurance (NHI).
Participants emphasized the urgent need for standardised employment, fair pay, and integration of CHWs into the public system, alongside strong protections for health and safety.
The conference called for a “People’s NHI” rooted in equity, solidarity, and worker participation, urging government accountability, union unity, and legal reforms aligned with international labour and health standards.
